  • Develop the confidence and skills required to coordinate and manage their support.

Levels of Support Coordination:

There are three main levels:

  • Support Connection: Designed to boost one’s capacity to connect with a variety of supports, be it informal, community-based, or funded.
  • Support Coordination: Focuses on building skills needed to understand and effectively use the plan. A coordinator works closely with the participant to ensure a mix of support, fostering increased independence, relationship maintenance, service task management, and community inclusion.
  • Specialist Support Coordination: A heightened level of support targeting those with more intricate situations requiring specialised assistance. It’s particularly useful for addressing challenges within the support environment and ensuring service consistency.

Inclusion in an NDIS Plan:

Funding is determined based on what is deemed ‘reasonable and necessary’. Support coordination, when justifiable, is included in the Capacity Building budget. The specific level of funded support coordination should match what’s described in the plan, ensuring participants select what best fits their needs.

 

Specialist Support Coordination: Going a Step Further

This is a more intensive coordination approach aimed at mitigating complexities in a participant’s support environment and assisting with major barriers in plan implementation.

  • Addressing complex barriers: They identify and comprehend the challenges that hinder participants from accessing, using, and maintaining their supports. Solutions are then sought and applied, with processes established to overcome these barriers systematically.

     

  • Designing service plans for complex needs: When significant complexities arise in a participant’s life, a detailed service plan becomes necessary. This involves crafting a plan that ensures all services operate harmoniously, enabling the participant to achieve their goals. It also lays down processes for efficient communication, problem resolution, and support access during crises.

What are some examples of complex barriers that a Specialist Support Coordinator might help a person with a disability to address?

  • A person with a disability who has multiple disabilities and needs support from a variety of different providers
  • A person with a disability who has a history of trauma and needs support to build trust with new people
  • A person with a disability who lives in a rural or remote area and has limited access to services
  • A person with a disability who has complex behavioural needs and needs support to manage their behaviour

Who is eligible for Support coordination and Specialist support coordination?

Anyone with an NDIS plan can be eligible for support coordination or specialist support coordination, depending on their needs. However, specialist support coordination is typically only funded for people with disabilities who have very complex needs.
